Welcome to Lambkin Athletics! 

Fort Collins High School Athletics have a long tradition of success.  Competing at the highest level in the state of Colorado, we have won 66 state championships throughout the school’s history.  Our athletic programs have also won over 300 conference championships, securing FCHS as the, “Home of the Champions.”  Our student body, staff, and alumni take great pride in carrying on the traditions and successes that past Lambkins have had and aim toward continued success for this year and into the future.  We are very fortunate to have outstanding student athletes and a very dedicated and passionate coaching staff to lead us to continued success in academics and athletics. 

We would like to thank all parents, coaches and athletes for your patience through this very uncertain time.

We are working very hard to get the most accurate and up to date information out to you as we get it. All forms that are posted are the most current information we have and will be updated as any new information comes in.

What you need to do to play a sport at Fort Collins High School

An Orange Card (mandatory practice/participation card) is required in order to participate in High School Sports. The Orange Card represents to the coach that all steps have been completed to participate. To obtain one complete the following steps.

An sports physical is valid 1 year from the date of the physician's signature. If unsure call the Athletic Secretary.

If you are enrolled at FCHS for the current school year

  1. Current Sports Physical Form (signed and dated by your doctor) 
  2. Complete your Athletes Online Registration Forms
  3. Register for a Big Teams/Planet HS account. Both athlete and parent must create accounts and link them. (This only has to be done for the 1st sport, per athlete, and will be used for all sports throughout high school)
  4. Pay the Athletic Fee (Each sport, every season will have it's own fee)
  5. Print and fill out the FCHS Athlete Information Page (Turn this completed page in to the Athletic Secretary)
  6. ​Please complete the above steps in order. Then proceed to the following steps to acquire an Orange Card [mandatory practice/participation card]
  7. Bring in:
    1. Current sports physical (if you don't have a current one on file at the athletic office) 
    2. FCHS Athlete Information Page
    3. Proof of Payment
  8. Bring Orange Card to Coach by the first official day of practice (failure to do so will result in you not being able to participate in a sport)

If you are not enrolled at FCHS for the current school year

  1. If you have not been placed by the PSD Athletic Office yet, contact Lori Willadsen (District Athletic Secretary) 970-490-3332 to complete this process.
  2. Current Sports Physical Form (signed and dated by your doctor)
  3. Print and fill out the  Athlete Registration Forms
  4. Register for a Big Teams/Planet HS account. Both athlete and parent must create accounts and link them. (This only has to be done for the 1st sport, per athlete, and will be used for all sports throughout high school)
  5. ​Please complete the above steps in order. Then proceed to the following steps to acquire an Orange Card [mandatory practice/participation card]
  6. Bring in:
    1. current sports physical (if you don't have a current one on file at the athletic office)
    2. Completed FCHS Athlete Registration Form
    3. Payment (cash, check or credit card)
  7. Bring Orange Card to Coach by the first official day of practice (failure to do so will result in you not being able to participate in a sport)

If you are a Transfer Student for the current school year

  1. Register for a Big Teams/Planet HS account. Both athlete and parent must create accounts and link them. (This only has to be done for the 1st sport, per athlete, and will be used for all sports throughout high school)
  2. Student or Parent begins the Transfer process by selecting the TRANSFER FORM button on their home screen
  3. Student verifies Name, Date of Birth, and grade; then signs the form digitally
  4. Parent verifies the following:
    • Phone number
    • Former address
    • Enrolled period
    • Previous school
    • Application choice
    • 2 questions about residence
    • Sport selection
    • Agreement of transfer terms
    • Email and name of sending school Athletic Director
  5. Parent signs completed form digitally, then submits
  6. Once complete, please notify the FCHS Athletic Director - Brian Poynter (bpoynter@psdschools.org) 970-488-8001 - that your form is ready for review/approval
